Penile & Pubic Surgery After Weight Loss
Losing a significant amount of weight is one of the most important things you can do for your health. But for many men, that achievement comes with an unexpected consequence: excess skin and tissue in the pubic area that conceals the penis and affects daily function, hygiene, and confidence. Board-certified plastic surgeon Mark P. Solomon, M.D., F.A.C.S., has over 35 years of experience in body contouring and male reconstructive surgery, helping men complete their transformation with results that match the effort they have already put in.
Dr. Solomon is an exceptional surgeon and a true professional. From the initial consultation through surgery and post-op follow-ups, he was clear, thoughtful, and incredibly attentive. He took the time to walk me through every detail of the procedure and made sure I felt fully comfortable going into surgery.
What Is Penile & Pubic Surgery After Weight Loss?
After significant weight loss, whether from bariatric surgery, GLP-1 medications (such as Ozempic, Wegovy, or Mounjaro), or personal effort, many men are left with excess skin and tissue in the pubic and abdominal area that hangs over and conceals the penis. This condition, known as buried or hidden penis, can interfere with urination, hygiene, intimate function, and self-confidence. Penile and pubic surgery after weight loss addresses these issues by removing excess tissue, recontouring the pubic area, and restoring a visible, functional, natural-appearing penis.
Procedure Overview
Massive weight loss is a major health achievement, but the skin that was stretched during years of carrying excess weight does not always shrink back. In the pubic region, this loose skin can fold over the penis, trapping moisture, causing irritation and infection, and making the penis appear significantly shorter than it actually is. For some men, the excess tissue makes standing urination difficult or impossible and creates barriers to sexual function.
Dr. Mark P. Solomon, M.D., F.A.C.S., is a double board-certified plastic surgeon with over three decades of experience in both body contouring and male reconstructive surgery. This combination of skills is essential for post-weight-loss penile and pubic surgery, where the surgeon must manage large areas of excess skin, sculpt the pubic fat pad, reposition penile shaft skin, and address scrotal changes, all while preserving sensation and function.
Double board-certified by the American Board of Plastic Surgery and the American Board of Surgery. Fellow of the American College of Surgeons (FACS). Over 35 years of specialized experience in male enhancement surgery.
Pubic Lift
The pubic lift is the cornerstone procedure for most men dealing with a buried penis after weight loss. It removes excess skin and fat from the pubic area, repositioning the remaining tissue to reveal the penile shaft. Dr. Solomon places the incision in the natural fold between the abdomen and pubic region, producing a discreet scar similar to a C-section line. This plastic surgery approach avoids the complications of urologic escutcheonectomy, which removes all pubic skin and can cause distortion and shortening.
Suction Lipectomy
When excess fat in the pubic pad contributes to a buried penis, liposuction can reduce the fat volume and improve penile visibility. This may be performed as a standalone procedure for men with primarily fatty tissue, or combined with a pubic lift for men who have both excess fat and excess skin.
Skin Fixation and Shaft Stabilization
After massive weight loss, the attachments between the penile shaft skin and the underlying tissue are often weakened or lost. This allows the skin to slide forward and telescope over the head of the penis. Dr. Solomon corrects this by placing fixation sutures that anchor the skin to the fascial layer of the penis, preventing telescoping and keeping the shaft visible. This technique preserves the penile skin, nerves, blood vessels, and lymphatics rather than removing them.
Abdominoplasty (Tummy Tuck)
In many post-weight-loss patients, the excess tissue extends well beyond the pubic area into the abdomen. When a significant abdominal skin apron (pannus) overhangs the pubic region, an abdominoplasty may be recommended alongside pubic surgery to achieve a proportional, comprehensive result. Addressing both areas in a single operation avoids staged procedures and creates a more natural contour from the abdomen through the pubic area.
Scrotal Surgery
Weight loss can also result in excess scrotal skin and scrotal webbing (penoscrotal webbing), where scrotal tissue extends along the underside of the penile shaft. Dr. Solomon can correct scrotal redundancy and webbing during the same procedure, improving the overall appearance and function of the genital area.
Additional Procedures
Depending on the patient’s anatomy and goals, post-weight-loss penile and pubic surgery may be combined with:
Penile Lengthening
Release of the suspensory ligament to provide an additional one to two inches of visible length, maximizing the result of the pubic lift.
Gynecomastia Surgery
Male breast reduction is common after massive weight loss and can be performed at the same time for men who wish to address both concerns in a single operation.
Body Contouring
Thigh lift, arm lift, or other post-weight-loss body contouring procedures may be coordinated as part of an overall treatment plan.
A personal consultation with Dr. Solomon is required to determine which procedures are appropriate for your individual needs and goals.
Benefits & Risks of Penile & Pubic Surgery After Weight Loss
For men who have achieved massive weight loss, penile and pubic surgery can be the final step in completing their transformation. As with any surgical procedure, it involves both potential benefits and risks. Dr. Solomon believes that informed patients make the best decisions.
Benefits
- Restored penile visibility: Surgery removes the excess skin and tissue that conceals the penis, revealing the shaft and producing an immediately noticeable improvement in apparent length
- Improved function: Many men regain the ability to urinate standing, maintain proper hygiene, and engage comfortably in sexual activity
- Permanent results: The correction is lasting, provided you maintain a stable body weight after surgery
- Resolution of chronic skin issues: Excess skin folds in the pubic area trap moisture and cause irritation, rashes, and infection; removing this tissue eliminates these recurring problems
- Proportional body contour: By addressing the pubic area (and abdomen when indicated), surgery creates a more balanced, natural appearance that matches the rest of your post-weight-loss body
- Comprehensive single-stage approach: Dr. Solomon addresses pubic skin, pubic fat, skin telescoping, scrotal changes, and abdominal overhang in a single surgical plan when possible, reducing the need for multiple operations
- Experienced specialist: Over 35 years of focused experience in body contouring and male reconstructive surgery, with specific expertise in the post-weight-loss patient population
Risks & Considerations
- Surgical risks: As with any surgery, there is a risk of infection, bleeding, adverse reaction to anesthesia, and delayed wound healing; post-weight-loss patients may have a somewhat higher risk of wound healing complications due to changes in skin quality
- Scarring: Incisions are placed in discreet locations, but all surgery produces scars; scar management protocols help minimize visibility over time
- Swelling and bruising: Post-operative swelling is expected, particularly in the pubic and scrotal area, and may take several weeks to months to fully resolve
- Temporary numbness or sensitivity changes: Some patients experience temporary changes in sensation; this typically improves as healing progresses
- Weight stability required: Significant weight regain after surgery can compromise results; maintaining a stable, healthy body weight is essential for a lasting outcome
- Drain management: Some patients require surgical drains for the first few days; Dr. Solomon's team provides clear instructions and removes them promptly
Individual results vary. All surgical procedures carry inherent risks. A personal consultation with Dr. Solomon is required to determine whether a procedure is appropriate for your goals and to discuss your specific risk profile.
A special thanks to Dr. Solomon and his staff for putting up with my endless questions both before and after surgery. I just had my post op photos taken and am even more pleased than I thought when comparing with my before photos. I recommend Dr. Solomon to anyone looking for a talented surgeon who will be completely honest with you and that is there for you every step of the way!! Thank you, Dr. Solomon!!
Am I a Candidate for Penile & Pubic Surgery After Weight Loss?
Candidacy for penile and pubic surgery after weight loss is determined during a private consultation with Dr. Solomon. In general, good candidates include men who:
- Have achieved significant weight loss through bariatric surgery (gastric bypass, gastric sleeve), GLP-1 weight loss medications (Ozempic, Wegovy, Mounjaro), or sustained personal effort
- Have excess skin in the pubic area or abdomen that hangs over and conceals the penis
- Experience difficulty urinating, maintaining hygiene, or engaging in sexual activity because of excess pubic or abdominal tissue
- Have reached a stable body weight and are not planning further significant weight loss
- Have scrotal webbing or excess scrotal skin contributing to a buried or hidden penis
- Are in good overall health and are medically cleared for surgery
- Have good control of any underlying conditions such as diabetes or high blood pressure
- Have realistic expectations about what surgery can achieve
Timing is important. Dr. Solomon recommends that patients reach a stable weight before proceeding with surgery, as additional weight loss after the procedure can create new areas of excess skin. If you are still in an active weight loss phase, Dr. Solomon can consult with you about the right timeline to plan your procedure.
During your consultation, Dr. Solomon will evaluate the specific factors contributing to your condition, explain which combination of techniques is appropriate for your anatomy, and develop a surgical plan designed to achieve the most complete result. He will also discuss whether insurance may help cover a portion of the cost if the condition is causing functional impairment.
Recovery & What to Expect
Surgery is typically performed on an outpatient basis under general anesthesia. Recovery varies depending on the extent of the procedures performed, but the general timeline below applies to most patients.
Recovery Starts with Rest
Plan to rest for the first several days following surgery. Swelling and bruising in the pubic, scrotal, and (if abdominoplasty was performed) abdominal areas are normal and expected. If surgical drains are placed, Dr. Solomon provides clear instructions for their care and removes them within days. Only dissolving sutures are used. Prescriptions for pain medication, antibiotics, and medication to minimize nocturnal erections during early healing are provided. Dr. Solomon sees his patients daily during the first few days after surgery. Out-of-town patients receive daily in-person check-ins at their hotel until they are cleared to travel home.
Light Activity & Early Healing Phase
Most men can shower within a day or two of surgery and return to desk work and light daily activities within one to two weeks. Swelling continues to improve gradually. Incision lines will be visible but will continue to fade significantly over the following months.
Returning to Exercise and Intimacy
Exercise and sexual activity are restricted for at least four weeks following surgery. For patients who had abdominoplasty in addition to pubic surgery, the restriction period may extend to six weeks. Dr. Solomon will clear you for full activity based on your individual healing progress.
Settling Into Your New Normal
The results of penile and pubic surgery after weight loss are permanent, provided you maintain a stable body weight. Scars take many months to settle and improve; Dr. Solomon provides guidance on scar management to optimize their appearance. Follow-up appointments, both in-person and virtual, are scheduled to monitor your progress and ensure your recovery stays on track.
Recovery timelines are general guidelines. Individual healing varies based on the procedure performed, overall health, and adherence to post-operative instructions.
Real Results. Real Patients
For men who have worked hard to lose weight, these results represent the final step in their transformation. Every outcome reflects a customized surgical plan developed specifically for that patient. We are proud to share these images with the permission of the patients themselves, who hope their experience will give others the courage to seek the care they deserve.

Frequently Asked Questions
When you carry significant excess weight, the skin and fat in your pubic and abdominal area stretch over time. When the weight is lost, the fat may decrease but the stretched skin remains. This excess skin hangs over the pubic area and conceals the penis. In addition, the skin attachments to the penile shaft are often weakened, allowing the shaft skin to slide forward and cover the head of the penis. The result is a penis that appears shorter, hidden, or buried, even though the penis itself is normal in size.
Yes. GLP-1 weight loss medications can produce substantial weight loss, and the resulting excess skin in the pubic area is the same regardless of how the weight was lost. If you have achieved a stable weight and excess tissue is concealing your penis or causing functional problems, you may be an excellent candidate for surgery. Dr. Solomon will evaluate your situation during a consultation.
Dr. Solomon recommends reaching a stable body weight before proceeding with surgery. If you are still actively losing weight, additional skin laxity may develop after the procedure, which could compromise the result. During your consultation, Dr. Solomon can help you determine the right timing based on where you are in your weight loss journey.
The specific procedures depend on your anatomy and the extent of excess tissue. Many patients benefit from a pubic lift (removal of excess pubic skin and fat), often combined with skin fixation to prevent telescoping. Some patients also need abdominoplasty (tummy tuck) to address abdominal overhang, scrotal surgery to correct webbing or excess scrotal skin, or penile lengthening via ligament release. Dr. Solomon will develop a customized plan after evaluating your situation.
Yes. The results are lasting, provided you maintain a stable, healthy body weight. Significant weight regain can create new areas of excess tissue and potentially compromise the correction. Dr. Solomon’s techniques are designed to produce a permanent outcome for patients who maintain their weight.
The surgery reveals the penile shaft that has been concealed beneath surrounding tissue, which means the visible length of your penis will increase significantly. If additional length is desired, Dr. Solomon can release the suspensory ligament at the same time, which may provide an additional one to two inches of visible length.
In some cases, yes. When buried penis causes functional impairment, such as difficulty urinating, chronic infections, or inability to engage in intercourse, insurance may classify the procedure as medically necessary. Dr. Solomon’s team has experience working with insurance companies to document medical necessity and help patients access coverage. Coverage varies by plan and by individual case.
Yes. Many post-weight-loss patients have excess skin in multiple areas. Dr. Solomon can coordinate penile and pubic surgery with abdominoplasty, gynecomastia correction (male breast reduction), thigh lift, arm lift, and other procedures. Combining procedures where medically safe can reduce overall recovery time and produce a more comprehensive result.
Most patients can shower within a day or two and return to light daily activities within one to two weeks. Exercise and sexual activity are restricted for at least four weeks (up to six weeks if abdominoplasty is included). Swelling resolves gradually over several weeks to months. Dr. Solomon provides detailed post-operative guidance and monitors your recovery closely, including daily in-person visits for out-of-town patients.
Absolutely. Dr. Solomon and his team treat every consultation with complete discretion and professionalism. The practice understands the personal nature of these concerns and maintains strict patient privacy. Virtual consultations are available for patients who prefer an initial conversation from home.
