How Much Does Buried Penis Surgery Cost With Dr. Mark Solomon?

The cost varies from patient to patient because no two cases are alike. That’s why we provide the exact pricing only after you consult with Dr. Solomon. We also don’t send quotes by email. For your visit, prepare $350 for the consultation fee.

That said, the total cost of a buried penis surgery with us typically covers:

  • Surgeon’s Fees: Reflect Dr. Solomon’s expertise, reputation, and track record
  • Facility and Operating Room Costs: Cover the nursing care provided and the equipment and medications used during surgery
  • Anesthesia Fees
    • Include care from our anesthesiologist, who administers anesthesia to keep you pain-free and monitors you throughout the procedure.
    • Costs vary depending on the type of anesthesia used (general, local, or regional) and the nature and duration of the surgery.
  • Post-operative Care
    • Includes follow-up visits, wound care, dressing changes, and management of any complications
    • At reputable practices like ours, follow-up visits often involve coordinated care with other specialists as needed.

If you’ve consulted other surgeons before, you may wonder why costs tend to vary significantly per provider. Here are two primary reasons for the price difference:

  • Facility and Geographical Location
Accredited centers in highly developed cities, such as Beverly Hills, California, may cost more but offer greater safety and comfort.
  • Surgeon’s Expertise
Board-certified specialists, such as Dr. Solomon, may charge more due to their advanced training and clinical experience.
Dr. Solomon, a fellow of the American College of Surgeons (FACS), has over 30 years of experience in the field. His proven credentials and decades of clinical expertise reflect the long-term value he provides.

Dr. Solomon believes patients deserve clarity, not surprises. During your consultation, you’ll receive a clear and detailed cost estimate tailored to your specific case, with no hidden fees or unexpected add-ons.

If insurance can cover part of the procedure, our team will guide you through the process, handle the paperwork, and help you determine the amount of coverage you’re entitled to.

What Are the Factors That Affect Buried Penis Surgery Pricing?

There is no flat sticker price for buried penis surgery. The final number depends on a mix of factors, including:

  • Case Complexity and Techniques Used During Surgery
Men with a buried penis may need more than one surgical technique.
Some men may only require suction lipectomy (surgical removal of fat using a specialized cannula). Others require more involved procedures, such as a tummy tuck (abdominoplasty), escutcheonectomy (pubic lift), or correction of webbed scrotal skin.
If there’s scar tissue or the patient has been diagnosed with severe obesity, surgery may take longer and require more resources, which can raise the cost.
  • Surgeon’s Experience and Expertise
A surgeon’s fee reflects their training, education, and skill in the operating room. It also covers practice expenses, including office costs, insurance, and staff salaries.
Dr. Solomon earned his medical degree from the New York University School of Medicine (one of the top medical schools in the world), completed his residency at the Hospital of the University of Pennsylvania, an Ivy League institution, and completed advanced fellowship training at Hôpital Necker-Enfants Malades in Paris, France.
He has since built his reputation as a pioneer in male reconstructive and aesthetic surgery. With over 30 years of practice and double board certification, his expertise is unmatched.
That level of specialization may mean higher fees, but it also means you’re paying for safer surgery and better outcomes.
  • Place of Surgery
The price of surgical procedures, like buried penis surgery, varies by location.
Dr. Solomon operates in Beverly Hills, California.
Costs in this area are typically higher than in smaller towns due to higher operating expenses and the demand for high-profile medical services.
  • Type of Facility
Accredited ambulatory surgery centers may command higher facility fees than basic suites due to stricter safety protocols, state-of-the-art equipment, and specialized care.
Dr. Solomon performs procedures at K&B Surgical Center, an AAAHC-accredited facility. This recognition from the Accreditation Association for Ambulatory Health Care (AAAHC) reflects our dedication to high-quality patient care.
  • Additional Procedures
Many men combine buried penis surgery with other treatments, like gynecomastia correction (male breast reduction) or penile lengthening (ligament release).
These additional procedures increase the upfront cost but consolidate recovery into one operation.

Deciding to move forward with buried penis surgery should feel straightforward, not stressful.

That’s why our office will help you find a payment plan that works for your situation.

We offer flexible financing options that let you manage the cost of treatment in affordable monthly installments.

Does Insurance Cover Buried Penis Surgery?

Insurance companies often draw a hard line between cosmetic and reconstructive (medically necessary) surgery.

Enhancements to length or girth are almost always considered cosmetic and are not covered.

But when surgery restores basic function, like solving problems involving urination, infections, or chronic pain, buried penis surgery may qualify as a medical necessity.

Approval usually comes down to whether your condition causes measurable problems in daily life. Insurers may cover surgery if you’re dealing with:

  • Trouble urinating (dribbling, spraying, or not being able to go while standing)
  • Urinary tract or skin infections
  • Painful erections or difficulty having sex
  • Hygiene problems that lead to irritation or skin breakdown

That said, insurance companies won’t just take your word for it. To build a strong case, you may need to prepare:

  • Medical documentation of your condition and a recommendation from Dr. Solomon or your primary doctor
  • Photos taken during the consultation that clearly show the condition
  • Proof that conservative fixes, like weight loss or hygiene routines, didn’t solve the problem

The good news is that you don’t have to stress over the insurance process. Our office takes the paperwork off your shoulders. We’ll help you determine insurance benefits.

What Is a Buried Penis and Why May Surgery Be Needed?

A buried penis, also called a hidden or concealed penis, is a condition where a normally developed penis is covered by skin, fat, or tissue from the abdomen, pubic area, or scrotum.

As a result, the penis looks smaller or invisible from the outside. In reality, the penis isn’t shortened, but “buried” beneath excess tissue.

This condition can be classified as:

  • Congenital (present at birth): Caused by abnormal attachments of the penis or extra folds of skin and tissue.
  • Acquired (develops later in life): Most often linked to obesity, drastic weight changes, scarring, or skin and lymphatic conditions, like lymphedema (swelling caused by a buildup of lymph fluid), balanitis (inflammation of the head of the penis), or lichen sclerosus (a chronic skin condition affecting the genitals).
A buried penis can also occur as a complication after surgeries such as circumcision.

In typical male anatomy, the skin at the base of the penis is anchored in place, so the whole shaft extends outward and remains visible. Usually, about one-third of the penis sits inside the body, and the remaining two-thirds are visible.

With a buried penis, excess skin, fat, or underlying tissue can fold over and “swallow” part, or sometimes all, of the shaft.

In some cases, the skin pushes forward and covers the glans (the head of the penis), while in other cases, fat from the lower abdomen or pubic area folds over and conceals it.

This condition can lead to significant physical and psychological problems:

Physical Impacts

  • Urine blockage, which may cause dribbling
  • Higher risk of urinary tract infections (UTI)
  • Difficulty having penetrative intercourse
  • Pain during sexual intercourse
  • Ongoing skin problems such as infections, irritation, and challenges with hygiene
  • Constant moisture or inflammation, which can cause tissue damage, scarring, and, though rare, a slightly higher risk of penile cancer.

Psychological Impacts

  • Deep struggles with self-esteem, which can lead to feelings of anxiety or depression
  • Strain on intimate relationships due to embarrassment or sexual difficulties
  • Pulling away from social situations or avoiding medical help, which only adds to the emotional burden

A buried penis can happen at any age. It can appear in children and in adults. And it is more common than you might think.

That said, it’s seen most often in men who struggle with obesity or significant weight changes. Excess fat can engulf the penis, while loose skin after weight loss may hang over and cover it.

In the US, about 4 in 10 adults are considered obese, a known risk factor for developing an acquired buried penis.

While exact numbers remain unclear, cases appear to be increasing alongside rising obesity rates.

Unfortunately, because many men feel embarrassed or avoid seeking care, the condition is often underdiagnosed, meaning the actual prevalence may be higher than what’s reported.

People who struggle with the following condition may also likely acquire a buried penis:

  • Failed circumcision: Too much or too little foreskin is removed
  • Lymphedema: Ongoing swelling or recurrent inflammation in the genital area can also contribute to its development

In mild cases, weight management or better hygiene may help provide some relief. However, as the condition progresses, moderate to severe cases rarely improve without surgical treatment.

Nonsurgical options can’t remove extra skin, fat, or structural changes that hide the penis.

But here’s the good news: Surgery can correct these issues and restore both function and appearance.

With more than 30 years of experience in complex reconstructive and male enhancement surgery, Dr. Solomon has built a career handling cases that many surgeons may never encounter.

He’s widely published, with over 35 peer-reviewed papers and contributions to top surgical textbooks that other doctors rely on for reference.

His approach is always patient-focused. Whether in person or virtually, he ensures men feel supported from consultation through recovery.

How Does a Buried Penis Affect Daily Life?

A buried penis is far more than a cosmetic concern. It can create serious functional, medical, and emotional challenges that usually get worse without proper treatment.

Here’s how it can affect health and day-to-day life:

  • Urination Difficulties
    • When the penis is hidden under skin or fat, it’s difficult to control or direct the stream.
Urine can dribble or become trapped, leading to dampness in underwear even after using the bathroom.
Blockage or misdirected flow may leave the bladder feeling only partly emptied.
    • Trapped urine creates a breeding ground for bacteria, increasing the risk of infection.
  • Hygiene Challenges, Soft Tissue, and Urinary Tract Infections
    • When the penis is hidden, urine and sweat often collect, creating a damp environment that encourages bacterial growth and urinary tract infection.
    • Extra tissue or fat makes proper cleaning difficult, increasing the risk of skin irritation, odor, discoloration, and even skin breakdown (through maceration or softening).
  • Sexual Dysfunction
    • When the penis is trapped or poorly visible, mechanical problems can make erections difficult to achieve or maintain.
    • Tight or overhanging skin can create tension that makes erections uncomfortable or painful.
    • In severe cases, tissue coverage can make intercourse nearly impossible.
  • Related Complications
    • Abscesses and fistulas: Severe infections may progress to abscesses (pockets of pus) and, in rare cases, fistulas (abnormal connections between tissues).
    • Phimosis: Excessive tightening of the foreskin can lead to weak urine flow.
    • Lichen sclerosus: This condition is characterized by pale, white patches and can cause pain and irritation on the glans and foreskin.
  • Psychological Impact: Affected individuals may develop anxiety and depression due to their condition.

Left untreated, a buried penis condition can worsen these problems over time:

  • Cycle of damage: Constant moisture and skin irritation can trigger infections, scarring, and tissue tightening.
  • Symptom progression: What begins as mild concealment or irritation can escalate into significant anatomical changes, loss of functional length, recurring infections, and ongoing pain.
  • Worsening emotional impact: As the condition lingers, so does the toll it can take on one’s mental health.
Frustration and poor self-image may deepen into depression or anxiety, especially when intimacy or daily activities are affected.

What Causes a Buried Penis in Adults?

A buried penis is a complex condition that can develop due to various reasons. Here are the most common causes, how they contribute to the problem, and why surgical intervention may be necessary:

Possible CauseHow it Causes a Buried PenisWhy Surgery is  Needed
ObesityExcess pubic fat and skin cover the penis.Once a buried penis has developed, only surgery can fully uncover the penis and restore normal form and function.
Post-circumcisionRemoval of too much (or too little) foreskin can lead to skin sliding (telescoping) and covering the glans.Revision circumcision often makes the problem worse.
Proper surgical correction stabilizes the shaft skin and secures it to the correct anatomical position, preventing the penis from becoming buried again.
Lichen sclerosus (balanitis xerotica obliterans)Inflammation causes scarring of the penile skin that entraps the penis.Lichen sclerosus can occur alongside a buried penis, in which case genital reconstruction may be necessary to restore sexual and urinary function and improve quality of life.
Genital lymphedema (penoscrotal lymphedema or lymphedema in the penis and scrotum area)Chronic buildup of lymphatic fluid in the scrotum may bury the penis. If the skin is scarred from rare conditions like lymphedema, it must be removed and the penile shaft reconstructed.
Persistent infectionRecurrent fungal or bacterial skin infections in the concealed, moist genital area can lead to ongoing irritation, tissue damage, scarring, and skin tightening. When scarring occurs due to a persistent infectious condition, such as filariasis (a parasitic disease), surgery may be required to remove the infection and reconstruct the penile structure.
Dysgenic dartosNormally, the ventral skin of the dartos fascia (underside of the penis) is connected to Buck’s fascia, the dense layer that surrounds the erectile tissue.
In a buried penis, this attachment is weak, allowing the skin and dartos fascia to slide downward while the erectile bodies remain fixed in place.
The only lasting solution is surgery that anchors the skin to the underlying fascia or repairs ligament problems through reconstruction.
CongenitalDevelopmental issues may lead to a hidden penis. Some congenital cases self-correct, but when concealment persists, early intervention may be needed to prevent complications like infections, urinary problems, or psychosocial issues.

How Does Buried Penis Surgery Work With Dr. Mark Solomon’s Advanced Techniques?

Choosing to undergo buried penis surgery can feel overwhelming, especially if you don’t know what to expect from the procedure. But don’t worry. We’re here to support you and provide clarity every step of the way.

Here’s how our process usually works:

  1. Pre-Op Marking and Photos
You arrive and settle down at our accredited outpatient facility. Dr. Solomon makes precise surgical markings and takes clinical photos for planning and documentation.
After that, you’ll be escorted to the operating room (OR).
  2. Check-In and Anesthesia
An IV (intravenous line) is placed on you, you meet our anesthesiologist, and the procedure is done under general anesthesia.
Most patients remember nothing from the OR.
  3. Correcting the Causes
Depending on your case, Dr. Solomon may perform one or a combination of these surgical approaches:
    • Fat removal, if needed, using a specialized cannula (suction lipectomy)
    • Pubic lift (escutcheonectomy) with a low, discreet scar
    • Abdominoplasty if lower belly tissue is part of the problem
    • Fixation sutures to stop telescoping
    • Scrotal web release with modern techniques
    • Optional ligament release to increase penile length
Buried penis surgery doesn’t treat erectile dysfunction (ED).
For cases involving complex urinary conditions or erectile dysfunction, Dr. Solomon partners closely with experienced urologists to provide complete, well-rounded care.
  4. Closure of Surgical Wound
Some patients need surgical drains (small rubber tubes attached to a bulb that collect excess fluid).
If drains are required, Dr. Solomon explains precisely how to care for them and removes them a few days after surgery.
The good news is that he uses dissolvable sutures, so no follow-up stitch removal is needed.
  5. Built-In Safety Net
If hospitalization is required, Dr. Solomon holds privileges at Cedars-Sinai in Los Angeles, providing patients with access to advanced care.

With over 30 years of experience in the field, Dr. Solomon has earned global recognition as a leader and pioneer in male enhancement surgery.

His approach focuses on safety, function, and natural results.

Dr. Solomon takes a specialized approach to buried penis surgery, focusing on restoring both function and appearance while preserving sensitivity.

One core principle runs through his work: preserving native penile skin to maintain shaft skin and ensure a reliable blood supply.

Other surgeons may remove all shaft skin and replace it with a graft, but this often destroys sensation and doesn’t address excess pubic fat or skin.

Instead, Dr. Solomon repositions the excess skin, removes only the excess from the pubic area, and restores normal visibility.

The outcome isn’t just cosmetic tightening, but also reconstruction that restores both function and appearance through advanced techniques refined over decades.

What Surgical Techniques Are Used for Buried Penis Correction?

Dr. Solomon evaluates the specific causes of concealment, including fat overhang, redundant skin, telescoping shaft skin, scrotal webbing, or prior scarring.

Then, he tailors a combination of maneuvers to correct the actual drivers in each case.

This phase involves reviewing your medical history, performing a physical examination, documenting your condition, applying day-of markings, and collaborating with urologists when complex issues affecting urinary and sexual function require it.

Depending on your anatomy and goals, Dr. Solomon may recommend:

  • Suction lipectomy: Extracts fat cells around the penis and pubic region using a specialized cannula.
  • Pubic lift (escutcheonectomy): Removes excess pubic skin and fat through a low, discreet incision (similar to a C-section scar). C-section is a surgical delivery of a baby through an incision.
Unlike the urology approach of removing all pubic skin, Dr. Solomon’s method preserves tissue for better function and aesthetics.
  • Abdominoplasty (tummy tuck): Reduces abdominal fat
  • Panniculectomy: Removes excess skin and fat from the lower abdomen
  • Fixation sutures: Anchor the shaft skin to the fascial layer (deep tissue) to prevent “telescoping” or sliding back over the glans.
  • Scrotal web release (“turkey neck” correction): Frees the penoscrotal angle using updated methods that avoid heavy scrotal scarring
  • Optional suspensory ligament release: In select cases, it reveals one to two inches of additional external length.
  • Rare graft or flap scenarios: Only used when skin must be removed. If needed, a scrotal flap is preferred because it behaves more like penile skin. Routine grafting is not recommended.

How Long Does Buried Penis Surgery Take?

Surgery time may vary depending on the nature of the surgery and the complexity of your case.

That said, surgery is performed under general anesthesia, usually on an outpatient basis, meaning most men go home the same day.

The operating time is affected by:

  • Number of techniques involved: A simple fat removal is faster than combining a tummy tuck (abdominoplasty), pubic lift (escutcheonectomy), and ligament release in one session.
  • Scarring or prior surgery: Scar tissue from past circumcisions, infections, or skin conditions like lichen sclerosus (a chronic inflammatory skin disorder) may require slower, more precise dissection.
  • Obesity or significant weight loss: A heavy pannus (overhanging belly) or ample folds of excess skin require additional contouring and fixation work.
  • Added procedures: Some men choose to pair buried penis surgery with enlarged chest (gynecomastia) correction or penile enlargement. This extends the operation but means only one recovery period.

Overall, the length of surgery depends on the complexity of your case. Straightforward procedures are quicker, while correcting fat, skin, scar tissue, or scrotal problems takes more time.

With decades of experience, Dr. Solomon can safely combine advanced techniques in one session, making the process efficient and the results long-lasting.

Preparing for Buried Penis Surgery: From Consultation to Pre-op

No two men experience a buried penis the same way. That’s why Dr. Mark Solomon starts every case with a complete evaluation. In your consultation, he will:

  • Review your medical history to identify potential contributing factors, such as obesity, prior circumcisions, diabetes, or infections.
  • Do a physical exam to see how much of the penis is hidden and whether fat, skin, or scarring is part of the problem.
  • Document the condition with photos and surgical markings to plan your procedure.
  • Evaluate functional concerns such as urination issues, painful erections, or hygiene challenges.
  • Explain your options, whether that’s fat removal, pubic lift, abdominoplasty, ligament release, or scrotal web correction, so you know which plan best matches your anatomy and goals.

As part of our screening process to ensure patient safety, pre-operative assessments may include:

  • Blood work: For evaluating the overall health status
  • Electrocardiogram (ECG or EKG): Especially if you’re 50 or older, to assess the heart rhythm
  • Primary physician clearance: To confirm that you’re medically fit for surgery
  • Diabetes screening: To know whether you need to regulate your blood sugar
  • Blood pressure review: Hypertension should be well-controlled before surgery
  • Medication review: Stopping certain anti-inflammatory medications like ibuprofen and aspirin to reduce bleeding risk

Preparing for surgery can be nerve-wracking, which is why we provide clear pre-operative instructions to keep you confident and ready at every step:

  • Skip shaving. Avoid shaving any body area before surgery, as it can increase the risk of infection after the operation.
  • Stay clean. A regular shower before the procedure is enough for hygiene.
  • Follow fasting rules. Dr. Solomon’s team will give you exact instructions on when to stop eating and drinking before the procedure.
  • Manage medical conditions. Ensure that chronic conditions, such as diabetes or high blood pressure, are under control to support a safer recovery.
  • Adjust medications. You’ll be told when to pause certain blood thinners or supplements ahead of surgery.
  • Keep weight stable. Excess weight makes both surgery and recovery more challenging, so maintaining a healthy weight is crucial.

What Happens During a Buried Penis Surgery Consultation With Dr. Mark Solomon?

When you meet with Dr. Solomon for buried penis surgery, you’ll receive a thorough, personalized evaluation.

He identifies all the factors contributing to concealment and devises a plan that restores both function and appearance.

During consultation, Dr. Solomon performs a thorough physical exam that includes:

  • Suprapubic pad: Check for extra fat or loose skin in the pubic area that can press down and hide the shaft.
  • Flaccid visibility: Assess how much of the penis is visible when soft (for many men with the condition, only the tip shows).
  • Erect shaft skin: Evaluate if the shaft skin telescopes (slides over the glans), which can cause painful erections or worsen concealment.
  • Stomach and pannus: Examine whether an overhanging belly (pannus) or stretched skin due to weight change contributes to the problem.
  • Scrotum: Look for a scrotal web (“turkey neck”) that may need release during surgery.
  • Overall health: Review weight history, diabetes or blood pressure control, and prior surgeries or scars.
Dr. Solomon emphasizes maintaining stable medical conditions and a steady weight for optimal results.

After examination, Dr. Solomon develops a personalized plan. This may include:

  • Suction lipectomy to remove fat around the penis.
  • Escutcheonectomy (pubic lift) to remove redundant pubic skin or fat with a discreet low scar.
  • Abdominoplasty (tummy tuck) for a heavy stomach fold that conceals the penis more.
  • Penile fixation sutures to stop the shaft skin from telescoping.
  • Scrotal web release with modern techniques.
  • Optional ligament release for one to two inches of extra length when appropriate.

Regarding costs, we do not publish flat rates on our website. What you need to remember is that cost depends on:

  • Complexity of the case (single vs. combined techniques)
  • Facility and anesthesia requirements
  • Whether additional procedures are performed simultaneously with the buried penis surgery

Arriving prepared ensures your consultation addresses both medical needs and personal goals.

If you’re having your first appointment with Dr. Solomon, bring the information he needs to craft a personalized treatment plan:

  • A list of medications, including the supplements and over-the-counter drugs, you take
  • Medical history records
  • Weight history, mainly if bariatric surgery or weight-loss medications have been used
  • Notes on functional concerns, such as urination problems, recurring infections, or painful erections
  • Personal goals and questions about function, appearance, or interest in additional procedures

Dr. Solomon’s evaluation goes far beyond a standard exam. He carefully reviews the pubic pad, penile skin, stomach pannus, scrotum, and overall health to design a plan specific to your anatomy.

With decades of experience and published authority in male surgery, he is uniquely qualified to handle even the most complex buried penis cases, providing patients with clarity and confidence before moving forward.

Are You a Good Candidate for Buried Penis Surgery?

You may be ideal for the procedure if you experience functional or psychosocial problems, such as:

  • Difficulty urinating, dribbling, or recurrent infections
  • Painful erections or inability to have intercourse due to concealment
  • Hygiene challenges causing chronic irritation or skin breakdown
  • Low self-esteem and quality-of-life impairment

During consultation, Dr. Solomon evaluates:

  • Suprapubic fat pad (size and weight distribution)
  • Penile shaft skin (stability vs. telescoping)
  • Scrotal conditions (webbing, lymphedema, or excess tissue)
  • Abdominal pannus (stretched abdominal skin from obesity or weight loss)

Many health and physical factors can influence whether you’re a good candidate and how successful the results will be:

  • Medical conditions
    • Diabetes must be well controlled for proper healing, and high blood pressure should be stabilized before surgery.
    • Men with bleeding disorders may not qualify.
  • Weight changes
The most common causes of a buried penis in adults are weight gain (creating excess fat in the pubic pad), use of newer weight loss drugs, and massive weight loss (leaving stretched, redundant skin).
  • BMI (body mass index) and stability
    • Men with an elevated BMI can still be candidates, but Dr. Solomon emphasizes that maintaining a stable weight is crucial for long-term success.
    • Individuals with very high BMI may require weight loss for health and safety, but surgery is often still needed afterward to remove extra skin.
  • Surgical needs: Some men need only pubic contouring through a lift or liposuction. Others may require a combined tummy tuck (abdominoplasty) and pubic lift if abdominal overhang contributes to concealment.

As an internationally recognized expert in buried penis repair, Dr. Solomon regularly treats patients from across the US and abroad, including complex revision cases that demand his advanced skill.

What to Expect During Buried Penis Surgery Recovery

Recovery after buried penis surgery can seem daunting, especially if it’s your first time having the procedure.

Here’s what you can expect and how we guide you toward a safe, smooth, and steady recovery:

Hospital Stay

Most procedures are performed on an outpatient basis. Patients typically go home the same day.

If complications arise or additional monitoring is needed, Dr. Solomon has admitting privileges at Cedars-Sinai Medical Center in Los Angeles.

Recovery Timeline and Expectations

  • Days 1 to 2: Showering is usually safe within the first couple of days.
  • Early recovery: Mild to moderate pain is normal and managed with prescribed medication. Dr. Solomon often includes antibiotics, pain relief, and diazepam to ease discomfort from nighttime erections.
  • First days and weeks: Bruising and swelling start to fade. Small rubber drains (Jackson-Pratt or JP drains) may be placed to prevent fluid buildup. You’ll be instructed on how to care for them, and they are typically removed within a few days.

If surgical drains were placed, they’re typically removed within a few days.

  • Weeks 3 to 4: Light activity is fine, but heavy workouts and sexual activity are still off-limits.
  • Around 4 weeks: Most men can safely resume both exercise and sexual activity.
  • Months after: Scars mature and fade. Dr. Solomon provides aftercare instructions for scar management.

Complete healing takes a few months, with final results typically visible once the swelling has completely subsided.

Wound Care and Incision Appearance

  • Incisions: A pubic lift incision sits low, similar to a C-section scar, at the junction of the abdomen and pubic area.
  • Stitches: Dr. Solomon uses dissolvable sutures, so no removal is necessary.
  • Scar healing: Scars flatten and fade over several months. Dr. Solomon provides scar-care guidance to speed up recovery.
  • Hygiene: Maintaining good hygiene before and after surgery is crucial in preventing infection.

Activity Restrictions

  • Immediately after surgery: Rest and limit movement.
  • No shaving: Avoid shaving the area around the surgical site before and immediately after surgery, as this increases the risk of infection.
  • Sex and exercise: Hold off for about four weeks.
  • Driving: Wait until you’re off pain medication and cleared by Dr. Solomon.

After your procedure, we’ll schedule early check-ins to monitor the removal of the drain, incision healing, and watch for any signs of infection.

Your follow-up care will continue as your scars mature, ensuring your results last and you feel confident every step of the way.

How Long Will Buried Penis Surgery Recovery Take?

Dr. Solomon emphasizes that recovery isn’t just about the procedure, but also depends on your health and how closely you follow instructions. Key factors include:

  • Overall health: Men with controlled conditions, such as diabetes or high blood pressure, usually heal faster. Unmanaged health problems can slow down recovery or increase the risk of complications.
  • Body weight: Maintaining a stable weight reduces stress on incisions. Men who’ve lost a large amount of weight often require extra skin removal, which can prolong the healing process.
  • Smoking history: Tobacco is widely known to impair wound healing.
  • Following instructions: Sticking to Dr. Solomon’s guidelines, like avoiding shaving before surgery, caring for drains properly, and holding off on sex or exercise for four weeks, helps you stay on track with your recovery.
  • Extent of surgery: A simple liposuction heals more quickly than a combined approach involving a pubic lift, tummy tuck, and ligament release.

Dr. Solomon sees patients several times post-op: early (drain removal), intermediate (healing check), and later (scar maturation) to ensure results progress as expected. His techniques are designed for permanent outcomes.

He usually sees his patients daily for three to five days after surgery. To facilitate your recovery, we ask that you stay at one of several hotels that allow him easy access to you so he can provide care. He visits you in person. There is no need for you to travel to the office after surgery.

Does Buried Penis Surgery Hurt During Recovery?

Most men are surprised to find that buried penis surgery isn’t as painful as they imagined.

Pain is typically manageable with prescribed medication and follow-up care. Dr. Solomon uses a combination of medicines to keep recovery as smooth as possible:

  • Pain relief (standard analgesics): Strongly recommended to help control soreness around the incisions
  • Antibiotics: Prescribed to lower the risk of infection while wounds heal
  • Diazepam: Given to reduce discomfort from nighttime erections in the early healing phase

We understand that some discomfort is a natural part of the recovery process, but there are simple steps you can take to make healing smoother and more manageable:

  • Moving early: Light walking soon after surgery helps improve circulation and reduce stiffness.
  • Drain and wound care: If drains are placed (like small JP drains), Dr. Solomon shows patients how to manage them to prevent fluid buildup and pain.
  • Follow-up visits: Early check-ins allow Dr. Solomon to spot and treat issues like infections, bleeding, or scar tension before they become painful.

Dr. Solomon uses refined techniques designed to reduce trauma and improve comfort.

By preserving native penile skin and avoiding unnecessary grafts, he lowers the risk of added post-op discomfort.

Each patient also receives a tailored medication plan and close follow-up to keep pain controlled and recovery safe.

How to Recognize Post-surgery Complications

Patients should contact Dr. Solomon’s team immediately if they notice:

  • Swelling or bruising that looks excessive or out of the ordinary
  • Severe pain not relieved by prescribed medication
  • Ongoing bleeding or unusual fluid draining from incisions or drains
  • Signs of infection, such as fever, redness, or foul-smelling discharge
  • Urination problems

While most men heal without major problems, untreated or complicated cases can present risks:

  • Infections: Ongoing concealment can cause skin breakdown, hygiene issues, or urinary tract infections.
  • Painful erections: If telescoping skin isn’t corrected, erections may stay uncomfortable.
  • Phimosis: A tight foreskin can persist without surgical fixation.
  • Scarring: Although rare, heavy scarring may affect appearance or require revision.
  • Emotional impact: Unresolved functional or cosmetic problems can continue to harm confidence and self-esteem.

Meanwhile, here’s how you know you’re healing properly:

  • First to several days: Swelling, bruising, and soreness are expected.
  • Around 4 weeks: Pain and swelling ease significantly, and most men may have sex or resume exercising.
  • Months after the procedure: Scars soften, swelling resolves, and results stabilize.

What Are the Results of Buried Penis Surgery With Dr. Mark Solomon?

Men who undergo buried penis surgery often report functional and aesthetic benefits:

Physical and Functional Improvements

  • Being able to urinate standing without mess or dribbling
  • Better hygiene, with fewer rashes, odor, or infections
  • Relief from painful erections and a return to normal sexual activity

Aesthetic Outcomes

  • Restored visible length, as the shaft, once hidden under fat or skin, is exposed
  • A natural penile contour, preserved by keeping native skin instead of using grafts

Success Rates and Long-Term Outcomes

  • Durability: Results are permanent for most men when fat removal, skin excision, and shaft skin fixation are done correctly.
  • Satisfaction: Many describe the surgery as life-changing, restoring both function and confidence.
  • Stability: Results are long-term unless new issues, such as significant weight gain, cause concealment to recur.

Dr. Solomon measures success in three key ways. Objectively, by confirming restored shaft visibility, stable skin without telescoping, normal urination, and the return of sexual function.

From the patient’s perspective, listening to reports of reduced embarrassment, improved confidence, and satisfaction with appearance and performance.

And through follow-up care, by monitoring scars and healing across several post-op visits to ensure both functional and aesthetic goals are achieved.

Will Buried Penis Surgery Make the Penis Longer?

Buried penis surgery doesn’t create new length, but restores the natural length hidden by excess fat, skin, or scarring.

Many men appear shorter simply because the penis is concealed under a suprapubic fat pad or loose skin.

Once these issues are corrected, the shaft becomes visible again.

For men who want added projection, Dr. Solomon may also perform a suspensory ligament release, which can provide an extra one to two inches of projection. Without ligament release, the gain comes from exposure, not enlargement.

It’s essential to distinguish between revealing hidden length and actual enlargement.

Buried penis correction is about unburying the shaft: removing fat, trimming skin, and using fixation techniques to keep the penis exposed. True enlargement (changing actual length or girth) is a different procedure.

Dr. Solomon makes this distinction clear, emphasizing three goals:

  • Restore function: Ensuring that patients experience normal urination, better hygiene, and improved sexual activity.
  • Reveal natural size: Exposing what’s already there.
  • Improve confidence: Helping men feel their penis looks and performs as it should.

Are Buried Penis Surgery Results Permanent?

For most men, results from buried penis surgery are permanent when the underlying causes are fully corrected.

By removing excess fat and skin and placing fixation sutures (to stop skin from sliding back over the glans), Dr. Solomon provides a lasting structural repair.

That said, certain factors can influence long-term outcomes:

  • Weight changes: Gaining weight can cause new fat to accumulate in the pubic area, while significant weight loss may leave stretched skin that still requires removal.
  • Medical conditions: Poorly controlled diabetes, high blood pressure, or bleeding disorders can slow healing or affect scar quality.
  • Post-op compliance: Following instructions is critical. This means avoiding shaving to reduce infection risk, caring for drains properly, and holding off on sex or heavy activity for about four weeks.

Dr. Solomon emphasizes the importance of structured aftercare, which includes early visits for drain removal and wound checks, mid-phase visits for monitoring scar and penile function, and long-term visits to confirm stability.

This approach ensures results remain both durable and natural over time.

“Will My Penis Surgery Leave Scars?”

Yes, but scarring resulting from Dr. Solomon’s procedures is generally minimal and discreet.

When a pubic lift or escutcheonectomy is needed, the incision is placed low between the abdomen and pubic area, similar to a woman’s C-section scar.

If a tummy tuck (abdominoplasty) is performed simultaneously, a longer low abdominal incision may be added to address both belly and pubic overhang.

Notably, Dr. Solomon avoids unnecessary removal or grafting of shaft skin. He repositions existing skin to preserve sensation and maintain a natural look. That means most men won’t have visible shaft scars.

Still, in some cases, scarring is inevitable. To reduce this unwanted result, Dr. Solomon applies advanced plastic surgery methods:

  • Fixation sutures secure the skin of the shaft and prevent telescoping, eliminating the need for skin grafts that can leave scars and compromise sensitivity.
  • Incision placement is kept in natural creases, like the pubic fold or low abdomen, never across the mid-pubic area where scars are more visible.
  • Only dissolvable sutures are used, sparing patients from the irritation and scarring of external stitches.
  • Use of modern scrotal web release techniques, as these have replaced older z-plasty methods, producing cleaner and less visible results at the penoscrotal angle.

With Dr. Solomon’s technique, scars are placed low and discreet, native skin is preserved to protect both look and sensation, and scars steadily fade with time, leaving most men with results that appear natural and feel normal.

What Are the Risks and Complications of Buried Penis Surgery?

Like any major operation, buried penis surgery carries some risks. They’re uncommon, but patients should be aware of possible complications:

  • Infection: Surgical sites can become infected if healing or hygiene is compromised.
  • Bleeding or hematoma: Blood may pool under the incision, especially if the patient resumes strenuous activity too soon.
  • Scarring: All surgery creates scars, but in rare cases, scars may heal thick or widened (hypertrophic scars). While these thick, raised scars aren’t life-threatening, they can cause itching, pain, or limit your movement.
  • Wound separation (dehiscence): Incisions can pull apart if under too much tension or if healing is impaired.
  • Damage to nearby structures: Careless technique could harm surrounding tissue in the pubic, scrotal, or penile area.
  • Positioning or nerve injury: Rare but possible during lengthy procedures under anesthesia.
  • Revision surgery: Some men may require a secondary procedure to address contour, tightening, or functional issues.

While risks can’t be eliminated, Dr. Solomon’s refined techniques and long track record in complex reconstruction make him one of the most experienced surgeons worldwide in buried penis repair.

His focus on preserving function, planning precisely, and guiding recovery significantly reduces complication risk while maximizing safety and long-term results.

How Dr. Mark Solomon Minimizes Buried Penis Surgery Complications

Dr. Solomon reduces complication risk through:

  • Decades of specialization: Over 30 years in male reconstructive surgery, with 35+ published papers and recognized leadership in the field.
  • Case-by-case planning: Each operation is tailored after evaluating fat, skin, abdominal pannus, and scrotal webbing.
  • Avoidance of routine grafts: Preserves and repositions native shaft skin for better sensation, appearance, and fewer complications.
  • Discreet incisions: Placed low in natural folds for less scar visibility and tension.
  • Dissolvable sutures: Eliminate the need for stitch removal and reduce the risk of wound separation.
  • Specialist collaboration: Works closely with anesthesiologists and urologists when urinary or sexual function requires additional care.
  • Close follow-up: Early monitoring detects infections, bleeding, or concerns about healing before they become serious.

Dr. Solomon avoids shortcuts like revision circumcisions or removing all pubic skin, which can worsen concealment and distort appearance.

Instead, he selects the right mix of techniques, such as liposuction, pubic lift, abdominoplasty, fixation, or scrotal web release, only when needed.

Many men come to him after failed surgeries elsewhere. His advanced methods are specifically designed for complex or revision cases.

All procedures are performed in accredited outpatient centers, with Cedars-Sinai hospital privileges available for more complex needs.

Dr. Mark Solomon’s Advanced Surgical Approach

  • Native skin preservation: Dr. Solomon emphasizes that most men with a buried penis have excess skin, not a shortage. Instead of removing the shaft skin and replacing it with grafts, he repositions natural skin.
This protects sensation, maintains blood and lymph flow, and creates a more natural look.
  • Fixation sutures: He pioneered methods to anchor penile skin to the fascial layer, preventing “telescoping” (skin sliding forward over the glans), which often causes concealment and painful erections.
  • Modern scrotal web release: Uses updated techniques instead of outdated z-plasty, freeing the penoscrotal angle with cleaner, less visible scarring.
  • Low-incision pubic lift (escutcheonectomy): Avoids the traditional urology approach of removing all pubic skin, which can distort or shorten the penis.
Instead, a discreet low abdominal incision gives a more natural result.
  • Dissolvable sutures only: Improves comfort and reduces wound complications by eliminating external stitches.
  • Comprehensive planning: Each surgery is tailored by evaluating fat, abdominal pannus, penile skin stability, and scrotal anatomy, and by addressing all factors that contribute to concealment.
  • Durable, lasting results: His techniques address the root cause, not just the appearance, resulting in stable, life-changing outcomes for most men.

With over three decades in male reconstructive and enhancement surgery, Dr. Solomon has refined approaches that minimize risk and maximize both function and appearance.

Men from across the US and abroad travel to consult him for complex or revision cases, often after failed surgeries elsewhere, because his methods consistently restore both form and function.

Related Male Enhancement Procedures and Costs

Many men choose to pair buried penis surgery with additional procedures that enhance length, refine scrotal appearance, or correct past complications:

  • Suspensory ligament release: For men seeking more visible length, Dr. Solomon can release the ligament that anchors the penis to the pubic bone. This usually adds one to two inches of external projection.
It’s important to note that buried penis surgery itself restores natural visibility but does not add new size.
Ligament release is the only option for lengthening performed during repair.
  • Penoscrotal web correction: Removes the “turkey neck” effect that hides the penoscrotal angle and can make intimacy uncomfortable.
  • Scrotoplasty (scrotum surgery): Reshapes stretched or bulky scrotal skin, improving both comfort and aesthetics.
  • Body contouring procedures: Liposuction can slim the pubic area, while abdominoplasty or panniculectomy can remove large abdominal overhangs that can worsen concealment.
  • Revision surgery: Corrects complications from prior circumcisions, failed penile operations, or poorly placed implants and fillers.

Patients often ask if these procedures can be performed together. The answer is yes.

Dr. Solomon frequently combines surgeries to correct multiple issues in a single operation and recovery period.

This approach saves time and cost while delivering more complete results: restored visibility, increased length, and a refined scrotal appearance in a single step.

With over 30 years of experience in male reconstructive and enhancement surgery, Dr. Solomon possesses the expertise to plan and execute these combination procedures safely and effectively, providing patients with lasting functional and cosmetic improvements.

Conservative Treatments vs. Surgical Options for Buried Penis

It’s natural to try conservative options before considering surgery. These may bring minor improvements, but they rarely solve the problem.

That said, here’s how nonsurgical measures compare to surgical solutions:

  • Weight Loss
Dropping weight can help trim fat around the pubic area and improve concealment.
But once the penis is already buried, weight loss rarely fixes the problem.
After significant weight loss, extra loose skin often sags over the penis, hiding it even more. In those cases, surgery is needed to remove or reposition the skin.
  • Penile Skin Incisions (Dorsal Slit or Revision Circumcision)
Some men are told the issue is just a “bad circumcision” and are advised to undergo a revision circumcision or a dorsal slit (an incision along the top of the foreskin).
Dr. Solomon warns against this. Cutting away more skin near the glans often backfires. The remaining shaft skin can slide down and bury the penis even deeper.
Do they help? Yes, weight loss can improve overall health.
But do they solve the buried penis? No, because the root problem usually involves excess skin, fat, or weak attachments, which cannot be corrected without surgery.

Here’s a summary comparing nonsurgical and surgical options:

  • Conservative Approaches
    • May reduce symptoms (less inflammation, slightly less concealment after weight loss).
    • Do not permanently restore penile exposure, function, or appearance.
    • If unnecessary circumcision revisions are involved, they may increase the risk of worsening the condition.
  • Surgical Approaches
    • Directly remove excess skin or fat (escutcheonectomy or pubic lift, abdominoplasty, suction lipectomy).
    • Restore penile visibility and function.
    • Can be combined with ligament release for added length.
    • Provide long-term, definitive results.
