Hidden But Not Forgotten: What You Need to Know About a Buried Penis

If you’re dealing with a buried penis, you likely know the anxiety and frustration that come with it.

Maybe you’ve looked in the mirror, hoping to see what should be there, only to be reminded that it’s hidden beneath excess skin or fat.

But it’s not just about looks. For some men, a buried penis can make everyday things—like urinating, staying clean, or having a fulfilling sex life—more difficult than they should be.

If you’ve already tried losing weight or maintaining better hygiene, and those efforts haven’t given you the results you hoped for, you may have questions like:

    • Will my penis keep disappearing?

    • Is there a real solution for this?

    • If surgery is the answer, is it worth it?

Let’s set the record straight:

A buried penis is more common than many think, especially in men struggling with obesity.

Your manhood isn’t shrinking—it’s just hidden beneath excess skin.

The good news? The right surgical approach can help.

Plus, you won’t be left to figure things out on your own—his team will guide you every step of the way, from consultation to post-op care.

It’s time to take back your confidence and improve your quality of life. Ready to make a change? Dr. Solomon is here to help.

What Is a Buried Penis?

, Hidden But Not Forgotten: What You Need to Know About a Buried Penis

A buried penis occurs when the penis is concealed beneath excess skin from the abdomen, thighs, or scrotum rather than being externally visible.

Despite appearances, the penis itself isn’t getting smaller or shorter—it’s covered by excess skin.

This disorder can occur at birth (congenital) or develop later in life (acquired), most commonly in men with severe obesity.

Many congenital cases undergo correction in childhood.

A buried penis can lead to difficulties with urination, sexual function, and self-confidence.

Dr. Mark Solomon partners with highly skilled urologists in Beverly Hills to treat any urinary or sexual function problems associated with a buried penis.

While Dr. Solomon specializes in the body contouring and reshaping aspects of buried penis surgery, he works closely with leading experts to ensure that you get the quality care you need and deserve.

What Causes a Buried Penis?

In buried penis syndrome, the shaft and head of the penis are hidden when the penis is viewed from the front or side.

There are several reasons this might occur. One is a condition in which the shaft skin slides down over the head of the penis.

Many of these men, in Dr. Solomon’s experience, have had one or two circumcision procedures.

A circumcision in a man with unstable shaft skin will pull more of the skin over the head of the penis and make things worse.

The correct maneuver is to pull the shaft skin back and stabilize it to the shaft of the penis to stop the sliding.

Another reason may be an excess of pubic skin or fat.

Men who present with a buried penis may have gained or lost a significant amount of weight through exercise and diet or after weight loss surgery.

Some may also experience this condition after using newer weight-loss drugs.

A buried penis may develop due to these additional factors:

    • Scrotal swelling due to lymph fluid buildup causing surrounding tissue to envelop the penis

    • Inflammation in your sweat glands (Hidradenitis suppurativa)

    • Inflammation of the head of the penis (balanitis)

This condition may result in pus development (abscesses) or an irregular connection (opening) between between body parts (fistula), which can cause a buried penis.

Still, obesity is the most common cause, and with nearly 40% of U.S. adults classified as obese, buried penis syndrome affects more men than many realize.

While exact statistics on acquired buried penis cases are lacking, the condition’s strong correlation with obesity makes it a growing concern.

If you’re worried about having a buried penis, your concerns are completely valid.

The good news? It’s treatable. And with Dr. Mark Solomon’s advanced surgical techniques, your manhood’s function and appearance can be restored.

No one should have to live with the discomfort and challenges of a buried penis. Let Dr. Solomon help you reclaim your confidence and quality of life.

How Is a Buried Penis Treated?

If there is excess fat only, liposuction, a basic plastic surgery procedure, can treat it.

If there is excess skin as well, then the best option is a pubic lift to remove the excess skin along with the fat.

Dr. Solomon can combine this procedure with a tummy tuck for men who need both procedures, or it can be performed as a single operation.

He can also perform a penis ligament release procedure for men who want to achieve greater flaccid penis length.

If you think you might have one of these conditions and would like more information, you may call Dr. Solomon’s office to schedule a consultation.

That said, there isn’t a one-size-fits-all solution for a buried penis—every case is unique.

However, several effective surgical techniques can restore both penile appearance and function, including the following:

    • Suction lipectomy: Removing fat cells from around the penis using a specialized cannula (a small tube used to remove fat cells).

    • Abdominoplasty: Reducing excess abdominal fat that may be contributing to penile concealment

    • In addition, the abdominal muscles can be tightened to enhance abdominal contour.

    • Escutcheonectomy (also known as pubic lift): Removing excess fat from the pubic region to expose more of the penis

While these procedures are commonly performed, Dr. Mark Solomon tailors each treatment plan to the patient’s needs, ensuring the best possible outcome.

Men who undergo buried penis surgery at our practice experience lasting results—restoring both function and self-confidence.

How Is a Buried Penis Diagnosed?

Like with any other medical problem, a buried penis condition’s nature must be understood, and the extent of any particular case must be evaluated to determine the ideal management practice.

Accurate diagnosis is necessary since not everyone has the same experience with the genital disorder.

That’s why Dr. Solomon and his team take a thorough, individualized approach to diagnosing a buried penis.

During your consultation, they will:

  • Review your medical history to check for underlying conditions that may contribute to the issue
  • Conduct a physical examination to assess how much of the penis is hidden and whether excess fat, skin, or other factors are involved
  • Evaluate functional concerns, like urinary tract infections (UTIs), painful erections, or hygiene challenges

Dr. Solomon will discuss alternative options if a bleeding disorder or another medical condition makes surgery potentially unsafe.

He always aims to ensure that only those who will genuinely benefit from the procedure move forward with treatment.

What Are the Symptoms of a Buried Penis?

A buried penis is a cosmetic and physiological medical problem. Common symptoms include the following:

  • Only the tip of the penis is visible
  • Urine dribbling due to poor stream control
  • Erectile dysfunction
  • Difficulty urinating while standing
  • Painful erections

The anatomy of an erection involves internal structures that extend deep into the pelvis.

Generally, about one-third of the penis remains inside the body, while the rest is externally visible.

That said, excess weight can shift this balance, concealing the penis even during an erection.

What Are the Possible Complications of a Buried Penis?

Leaving a buried penis untreated can lead to several physical and psychological complications, such as:

  • Painful erections that make intimacy and daily life uncomfortable
  • Difficulty keeping the penis clean, leading to skin irritation, discoloration, or even breakdown
  • Phimosis (an excessively tight foreskin)
  • Increased risk of penile cancer associated with chronic inflammation and hygiene challenges, which are consequences of a buried penis
  • Hard-to-spot skin conditions like lichen sclerosus, which causes itchy, white patches on the penis
  • Psychological distress due to the condition’s impact on one’s self-esteem and quality of life
“Will I Need to Do Anything to Prepare?”

If you’re considering buried penis surgery, proper preparation is vital to a smooth procedure and recovery.

The best way to ensure you’re prepared is to consult with a uniquely qualified, board-certified plastic surgeon, such as Dr. Solomon.

He can identify specific issues and develop a tailored care strategy.

  • First, you’ll need a medical evaluation from your primary doctor to confirm if you’re a candidate for the surgery.

You may need to undergo blood tests or an EKG (electrocardiogram), particularly if you are 50 or older.

  • If you have diabetes, managing blood sugar control is essential to facilitate recovery. Dr. Solomon will inform you regarding any necessary tests or assessments before the procedure.
  • Pre-op preparations may vary slightly from patient to patient. That said, this phase typically involves managing health conditions, including high blood pressure or diabetes.
  • You’ll have to maintain hygiene before the procedure, but avoid shaving any area of the body beforehand.

Shaving can increase the likelihood of postoperative infection.

Do you still feel uncertain about any of these steps? Dr. Solomon and his team will ensure you have clear, personalized instructions so you can feel confident and well-prepared for surgery.

“What Should I Expect on the Day of Surgery?”

Getting anxious before undergoing buried penis surgery is understandable, especially if it’s your first time.

Knowing what to expect on the day of the operation may help calm your nerves. Here’s a step-by-step breakdown of the procedure:

  1. Upon arrival at the facility, you’ll have an intravenous line or IV (a flexible, soft tube inserted into a vein) placed.
Dr. Solomon performs the surgery on an outpatient basis with the help of a highly skilled physician anesthesiologist.
This way, you’ll feel completely comfortable during the procedure.
You might not even recall what happens inside the operating room until you wake up.
Feel free to raise any concerns or questions regarding anesthesia with the anesthesiologist.
  2. Dr. Solomon may combine several procedures, depending on your situation.
For instance, if you have gynecomastia (enlarged breasts), Dr. Solomon can treat that at the same time through another surgery.
He may also perform scrotal or penis enlargement surgery (if you prefer). These procedures are ideally performed simultaneously with buried penis surgery.
  3. In some cases, small rubber drains may be placed to prevent fluid buildup, but they’ll be removed within days.
Dr. Solomon uses dissolvable stitches, so there’s no need for a follow-up appointment to remove them.
  4. Now, if your surgery includes a pubic lift, you may have a scar similar to a C-section incision (a surgical cut used to deliver a baby).
While scars take months to fade, Dr. Solomon will provide scar tissue care guidance to promote optimal healing.
Dr. Solomon doesn’t use a skin graft to cover the base of the penis or the head of the penis (glans).
“How Soon After Treatment Will I Feel Better?”

The recovery period for buried penis surgery varies, but most patients start feeling better within a few days. Here’s what to expect:

  • You can shower within a day or two after the procedure.
  • You’ll receive pain medication and other prescriptions, such as antibiotics and diazepam (to ease discomfort from nocturnal erections).
  • You may resume light physical activities within a few days. Still, exercise and sex are off-limits for about four weeks.
  • Dr. Solomon will monitor your recovery closely to observe signs of infection or complications. If hospitalization is necessary, he has privileges at Cedars-Sinai for next-level care.

From your first consultation to full recovery, we’re here to provide the expert support and compassionate care you deserve.

“What Can I Expect if I Have a Buried Penis?”

A buried penis can impact nearly every aspect of your life, from daily hygiene to self-confidence and intimacy.

Here’s what you might experience:

  • Intimate relationships could become complicated. Concerns about appearance and function can affect confidence in the bedroom.
  • Daily hygiene may become challenging. Keeping the genital area clean can be difficult, increasing the risk of infections and skin irritation.
  • Self-confidence is likely to suffer. Many men tie their self-image to their genital appearance, potentially leading to emotional distress.
  • Physical discomfort may persist. Urinary issues, painful erections, and general discomfort can interfere with everyday life.

It’s not just about looks—how a man feels about his body plays a huge role in self-perception, relationships, and overall well-being.

Looking down and not seeing your penis can feel like a bad dream, but the good news is that it’s treatable.

Dr. Solomon specializes in restoring both form and function, helping men reclaim their confidence and quality of life.

If you’re ready to explore your options, we’re here to guide you. Schedule a virtual consultation, or visit us at our Beverly Hills office.

Can a Buried Penis Be Prevented?

A buried penis isn’t just a pediatric condition—it can also be adult acquired, often due to excess pannus (overhanging lower abdominal skin) from obesity.

Maintaining a stable, healthy body weight is the best way to reduce the risk of developing a buried penis, especially since obesity is a leading cause.

While the ideal weight varies for each man, staying within a healthy range can help prevent excess fat from burying the penis.

Weight loss can sometimes improve the condition by reducing fat around the pubic area.

That said, in cases of extreme weight loss, excess skin can contribute to penile concealment, requiring surgical intervention.

Once a buried penis develops, there are no nonsurgical treatments to correct it fully.

“When Should I See a Healthcare Provider?”

If you have a buried penis, it’s vital to consult a healthcare provider, especially if it:

  • Causes urination problems, such as dribbling
  • Makes maintaining hygiene more challenging, resulting in skin irritation or infections
  • Causes recurring urinary tract infections
  • Affects sexual function

If your buried penis causes any of these problems, seeking medical help from a qualified surgeon like Dr. Solomon is the first step to healing and transformation.

“What Questions Should I Ask a Healthcare Provider?”

If you’re considering treatment for a buried penis, asking the right questions can help you feel more confident and informed.

Here are key questions to discuss with your healthcare provider:

  • What is causing my buried penis? (Is it weight-related, congenital, or due to scarring?)
  • What treatment options are available for my case?
  • Am I a good candidate for buried penis surgery?
  • What are the risks and benefits of surgery?
  • How long will recovery take, and what should I expect?
  • Will I need to make any lifestyle changes before or after surgery?
  • What happens if I don’t treat my condition?
  • How much will the procedure cost, and will insurance cover any part of it?

At our Beverly Hills practice, Dr. Solomon and his team take the time to answer all your concerns.

We ensure you feel heard, valued, and fully informed about your options—because your penile health and confidence matter.

FAQs

How many nights will I have to spend in the hospital?

Most men undergo surgical treatment for buried penis on an outpatient basis, meaning you’ll be able to go home the same day (no need to stay overnight at the hospital).

But if your surgical management is complex—especially if you have genital lymphedema, a large fat pad over the pubic area, or a history of skin infections—you might have to be monitored in person for several weeks.

Morbid obesity can also affect your recovery timeline, so in some cases, a short hospital stay is necessary to ensure you’re healing well.

What medications will I go home with?

You may be prescribed these medications to help with your recovery:

  • Pain medications to manage post-surgical discomfort
  • Antibiotics to prevent skin infections in the genital area
  • Diazepam or similar muscle relaxers to help control nocturnal erections
When will I need to come back to see my plastic surgeon?

Your first follow-up with Dr. Solomon takes place one day after the surgery to evaluate your progress and help you learn how to care for yourself at home. Dr. Solomon will monitor your progress daily for several days after surgery and then at weekly and monthly intervals for up to one year.

Suppose your condition involves urology concerns like urinary retention or sexual dysfunction. In that case, a urologist working alongside Dr. Solomon may monitor you over the next few months.

How will I know if I’m having a problem with my surgery?

Watch for signs like:

  • Excessive swelling or bruising beyond what’s expected
  • Severe pain that can’t be managed by your medications
  • Persistent bleeding or unusual drainage from the incision or drain sites
  • Infection symptoms like fever, redness, or foul-smelling discharge
  • Difficulty urinating

If you feel something is off, don’t hesitate to call the office immediately.

What will my incision look like?

The appearance of your incision will depend on the surgical option used to reconstruct the genital area and penile shaft.

Say you undergo a pubic lift (escutcheonectomy) or panniculectomy to remove excess skin.

In that case, the incision will likely be in the lower pubic area, similar to a C-section scar.

Dr. Solomon will reshape and improve the appearance of the pubic region to make the changes appear as natural as possible.

Post-surgery scars will be in a discreet place (between the pubic area and the abdomen).

And don’t worry—these incisions fade and eventually blend with your natural skin folds.

Will I have any tubes coming out of me after surgery?

Possibly. Some patients need surgical drains—tiny tubes that prevent fluid buildup around the penis repair site. If used, these are temporary and are removed within a few days.

If you have difficulty urinating, a temporary catheter may also be placed.

When will I know if I am cured?

Recovery isn’t just about physical exam results but also about quality of life.

For many obese men struggling with low self-esteem and mental health due to a buried penis, surgery is life-changing.

Physically, swelling subsides over several weeks, but the final results take months as the skin fully settles.

You’ll know you’re “cured” when:

  • You can urinate comfortably without obstruction.
  • Your sexual function has improved—whether that’s better sensitivity, confidence, or ability to engage in intercourse.
  • You no longer suffer from poor hygiene, penile skin infections, or discomfort from trapped moisture in the pubis.
  • Your mental health and self-image are better, knowing that your body now reflects what it should have been all along.

Few people understand the emotional and physical burden you carry as you go through life with a buried penis.

Whether looking at your reflection in the mirror or getting intimate in the bedroom, the feeling of embarrassment associated with how your penis looks can strike at the worst moments.

But you likely came here because you don’t believe the story is over—you’re still fighting for the chance to reclaim your self-image.

Dr. Solomon is the best ally you could ever wish for in your journey to healing and transformation.

His decades of expertise have helped many men like you achieve remarkable outcomes.

He’s highly skilled, genuinely cares about his patients’ well-being, and has a proven track record of easing your worries.

by Dr. Heather

Apr 5,2021

Hello! Ok, let me start by saying that Dr. Solomon is THE BEST. The reason I stand behind 5 stars is simple. I received a breast Augmentation 17 years ago and everything went beyond well. Not only did my breast look fabulous they still look fabulous after 17 years. I just left his offices for my 2nd check up in the past 17 years and they look and feel like they did when they were new. Not only did the check up cost ZERO dollars the but the cander and expertise are priceless. Look no further. Dr. Solomon is extremely honest and won't do anything that won't be perfect. Thanks DOC you Rock!
