Fat grafting is a process in which your fat is removed with liposuction and then placed into an area where additional fullness is desired, such as the buttocks, the breast or the face. Many surgeons also place fat in the penis. But unlike the buttock or the face, there is no fat in the normal penis. Skin has fat in its deepest layer in most of the body but not in the penis. In addition, fat grafts are soft. They also respond to changes in weight. So fat grafts in the penis can grow as men increase their weight with aging.
Fat grafts are small amounts of fat placed in different areas of the penis. Not all of the fat will last. On average about 30 percent of fat grafts don’t survive. Therefore, how you look right after fat grafting will not be how you’ll look in a year.
In plastic surgery that we replace like tissue with like tissue. In the penis, which normally has no fat, it’s not appropriate to add fat.
Removing fat requires surgery. It can’t be dissolved and liposuction may leave residual fat behind since the surgeon can’t see what’s being removed. Operations to remove fat require some experience since the fat can stick to the skin above or the nerve below. Fat can also shift with time and I’ve seen men with fat on the underside of the penis that ends up in the scrotum.
